Job Summary
Remote Population Health RN role focusing on non–face-to-face post-discharge outreach calls within CMS requirements, reviewing discharge plans, conducting medication reconciliation, educating patients on hypertension and follow-up care, coordinating with providers and care teams, documenting in the EHR, and ensuring timely follow-up and readmission-prevention activities. Requires an ADN (bachelor’s preferred) and an active RN license; remote work available for TN residents from Virginia Beach, VA.
Required Qualifications
- Associate degree in Nursing (ADN) or RN diploma/certificate with a current RN license recognized by the state
- 1 year RN experience
- RN license valid in the state of employment (compact/multistate considered)
