Job Summary
Remote TN-resident RN position handling post-discharge outreach calls to patients, reviewing discharge plans, coordinating care, educating patients on hypertension and follow-up, and scheduling care with providers. Requires nursing licensure, experience as an RN, and ability to document in EHR; role includes telephonic follow-up, care coordination across teams, and quality-improvement participation. Works under supervision of Nurse Manager with independent responsibilities in patient outreach, medication reconciliation, and ensuring appropriate follow-up and readmission-prevention efforts.
Required Qualifications
- Associate degree in Nursing (ADN) or RN diploma/certificate with current RN license
- One year RN experience
- Current RN license recognized by NCSBN Compact State or licensed in the state of work
- Bachelor's degree in Nursing preferred
