Job Summary
Remote RN for Population Health conducting post-discharge outreach via telephone, reviewing discharge plans and meds, performing medication reconciliation, assessing understanding and adherence, coordinating with care teams, scheduling primary care follow-ups, and documenting interactions in the EHR. Provides hypertension-focused education, identifies early signs of complications, supports readmission prevention, addresses care gaps (hypertension, diabetes, preventive screenings), and contributes to quality improvement initiatives. Requires nursing education so the nurse can educate patients and coordinate with clinicians, care coordinators, and case management while ensuring CMS and unit standards are met. The role emphasizes independent work with supervisory support and a focus on telephonic patient care and follow-up.
Required Qualifications
- Associate degree in Nursing (ADN) or RN diploma; current RN license
- One (1) year of RN experience
- RN compact/multistate license recognized by the NCSBN Compact State or licensed to practice in the working state
