Registered Nurse (RN) in Population Health, remote for Tennessee residents with onsite Albany, NY option. Responsibilities include non-face-to-face post-discharge outreach, review of discharge instructions and medications, medication reconciliation, assessing patient understanding and adherence, documenting interactions in the EHR, coordinating with care teams to address gaps, scheduling follow-ups, and contributing to quality improvement.
