Monitor, examine, and evaluate patient conditions while delivering care plans that promote short and long-term outcomes. Collaborate with the care team to coordinate daily patient flow, prioritize actions based on needs, and maintain accurate documentation of assessments and care changes. Serve as a patient advocate by listening to concerns, educating families on health issues, and ensuring patient-centered care standards are met. Participate in performance improvement initiatives, support shared governance councils, and mentor other nurses through preceptorship.
