Monitor, examine, and evaluate patient conditions while delivering care plans that prioritize short- and long-term outcomes. Collaborate with the care team to coordinate daily patient flow, maintain accurate documentation of assessments and care changes, and serve as an educational resource for patients and families. Listen to concerns, respond to needs in a timely manner, and contribute ideas to performance improvement initiatives aimed at better patient quality and staff satisfaction. Maintain professional licenses, complete required training, and foster growth through mentorship and peer feedback processes.
