Job Summary
Conduct comprehensive home health care plans by assessing patients, measuring vital signs, administering IVs, and changing dressings. Provide skilled nursing care, preventative rehabilitative procedures, and prescribed treatments across complex home situations while collaborating with physicians, therapy, and social services. Deliver patient and family education for disease management and health promotion as directed by the physician's plan. Complete case management duties and maintain the utmost standard of care. Work 8-hour rotating weekend shifts with a requirement to attend full orientation during the first week.
Required Qualifications
- New York State RN license
- Minimum of one year of recent hands on experience as a Registered Nurse
- Basic Life Support Certification
- Valid New York State Driver's License and reliable transportation
- 8 hour shifts (8:30AM to 4:30PM) - Rotating Weekends
- Per Visits are required to work at least 1 shift per week
- Per visits are required to attend entire first week of orientation Monday – Friday (8:30AM -4:30PM.)
Desired Qualifications
- CHHA experience strongly preferred
