Job Summary
Per diem Registered Nurse for Hospice Home Care in the Albany/Rensselaer County region. Coordinate with an interdisciplinary team to develop a comprehensive plan of care for patients and families in their homes. Work with a team of aides, LPNs, medical directors, NPs, social workers, chaplains, and other professionals. Availability is during weekdays (Mon-Fri), 8:00 a.m. – 4:30 p.m., with no weekends or holidays. Per diem rates based on hours; mileage reimbursement and orientation provided. Requires NY RN license, ADN (BSN preferred), and a valid driver's license with reliable vehicle. Prior hospice/home care or acute care experience is preferred; training and orientation provided.
Required Qualifications
- Current license to practice as a Registered Nurse in the State of New York
- Associates Degree in Nursing, BSN preferred
- 1 year experience as an RN, preferably in Hospice/Home Care or acute care
- Valid Driver's License and Reliable vehicle
