Job Summary
Provides in-home nursing care and develops/implements plan of care; assesses patient status, reevaluates needs, and revises care plans with patient and family involvement; administers medications per physician's orders; documents clinical notes and communicates changes to the physician; ensures a safe home environment; participates in interdisciplinary team meetings; participates in on-call duties; coordinates equipment and services; mentors/oversees ancillary personnel; pursues ongoing professional development and quality-improvement activities; requires strong clinical skills, BLS CPR, and a valid driver's license for in-home visits. 2+ years of nursing experience, preferably in geriatrics; degree in nursing from an accredited institution; proficient with EMR; strong communication and knowledge of healthcare laws and regulations.
Required Qualifications
- Degree in Nursing from an accredited institution
- Proficient with electronic medical records
- Excellent bedside manner and communication skills
- Working knowledge of health care laws and regulations
- BLS CPR certification
- Valid driver's license and own a reliable, insured vehicle
- 2+ years nursing experience (geriatrics preferred)
