Provide telephone-based nurse triage and clinical assessments across a multi-state program using established triage protocols, documenting interactions in client EHRs and coordinating referrals, prior authorizations, and follow-up care. Support Remote Patient Monitoring by reviewing vital signs and delivering education for chronic disease management; conduct follow-up outreach and educate patients as needed. Collaborate with providers and external entities (hospitals, nursing homes, departments of labor/MV) to ensure timely, accurate patient care, while maintaining HIPAA-compliant remote workflows. Possess strong teamwork, leadership willingness, customer service, and documentation accuracy, with ability to handle a minimum of four calls per hour during peak periods and manage multiple EHR platforms and telehealth tools.
