Job Summary
Conduct client assessments to develop individual care plans and deliver primary care in collaboration with healthcare professionals. Manage complex wounds, chronic diseases, IV therapy, dialysis, and medication while providing palliative and rehabilitative programs. Perform phlebotomy, specimen collection, and community outreach across home, clinic, and virtual settings. Requires full RN license, personal vehicle with valid license and insurance, current criminal record check, and ability to wear PPE. Schedule includes bi-weekly 60-hour blocks across days, evenings, and weekends with on-call rotation. Required Qualifications
- Must be licensed as a Registered Nurse in the province of practice
- Must enjoy driving
- Must have a personal vehicle
- Must possess both a current driver's license and proof of vehicle insurance
- A satisfactory Criminal Records Check is required
- A Vulnerable Sector Search and/or Child Abuse Registry Check may also be necessary
- Must be able to wear Personal Protective Equipment (PPE)
- This role requires a detail-oriented approach in a dynamic environment
- This role requires physical activity including lifting, carrying (using proper techniques), bending, reaching, kneeling, and other movements that emphasize good body mechanics
- Individuals in the role are required to walk, sit, stand, and climb stairs throughout the day
- Some tasks require fine hand movements
