Job Summary
Licensed Practical Nurse (LPN) or Registered Nurse (RN) sought to support the Rural Health Transformation Program at Southern Jersey Family Medical Centers, delivering and coordinating nursing services across clinic, mobile, RPM, and home-based settings in rural and medically underserved communities. Responsibilities include clinical care and program implementation aligned with grant objectives, patient-centered nursing care via Mobile Medical Unit outreach and follow-up home visits, care coordination and patient education, documentation and compliance with HIPAA and HRSA requirements, immunizations and injections within licensure, and collaboration across departments to support patient care delivery. The role emphasizes addressing social determinants of health, facilitating telehealth access, and ensuring quality outcomes in ambulatory/primary care and community health settings.
Required Qualifications
- Graduate of an accredited nursing program (LPN or RN)
- Active Licensed Practical Nurse (LPN) or Registered Nurse (RN) license in the State of New Jersey
- Current BLS/CPR certification
- Valid driver’s license and reliable transportation for local travel
- Ability to perform physical duties including standing for extended periods and lifting supplies/equipment as needed
- Strong computer skills and familiarity with medical terminology
- Excellent verbal and written communication skills
