Job Summary
Conduct comprehensive, patient-centered nursing care in an outpatient primary care setting, focusing on health promotion, disease prevention, and chronic disease management. Perform patient intakes including vital signs, health histories, and medication reconciliation; administer medications and immunizations; and execute procedures such as wound care, injections, and specimen collection. Monitor patients for acute and chronic conditions, coordinate referrals and diagnostic testing, and provide education on diagnoses, medications, and lifestyle changes. Accurately document assessments in the electronic health record while collaborating with physicians, providers, and the interdisciplinary team. Maintain licensure, participate in quality improvement initiatives, and work at least one evening per week in a fast-paced medical office environment.
Required Qualifications
- Registered Nurse or Licensed Practical Nurse License
- Currently registered as a Registered Nurse or Licensed Practical Nurse in the Commonwealth of Massachusetts and its Board of Registration in Nursing
- AHA BLS Health Care Provider certification or equivalent
- Required to work at least one evening per week
- Demonstrated written and verbal communication skills in English
- Normal visual acuity
- Standard clerical dexterity to operate computers, printers, photocopiers, and fax machines
- Moderate walking, standing, reaching, and stooping
Desired Qualifications
- Work experience equivalent to one year as an RN/LPN in an ambulatory medical office environment preferred
- Bilingual in Spanish and/or Portuguese is a plus
- Demonstrated ability to work in a fast-paced medical office environment
- Demonstrates understanding and commitment to the health center mission
- Demonstrates understanding and commitment to the established CHC Values and Standards
