Job Summary
Provide skilled, compassionate nursing care to hospice patients and collaborate with an interdisciplinary team to develop and implement individualized care plans. Support patients and families through education, comfort, and advocacy while documenting patient care and communicating changes in condition. This role includes on-call rotation for RNs, covering up to one shift per week from 4pm to 8am Monday through Thursday and every fourth weekend from Friday 4pm to Monday 8am. Travel within a one-hour radius is included with a company car provided for eligible employees. Required Qualifications
- Current RN or LPN license
- Excellent communication and interpersonal skills
- Valid driver's license and reliable transportation
- RNs are required to participate in the on-call rotation as part of this position
- This role includes on call coverage
- Up to 1 shift per week, Monday - Thursday 4pm to 8 AM
- Up to every 4th weekend, Friday 4 PM to Monday 8 AM
- Travel within a one-hour radius
- Company car is provided for employees working 0.6 FTE or more
