Job Summary

Assess patient needs upon initial encounter and periodically throughout navigation, matching unmet needs with appropriate services and referrals such as dietitians, social work, and financial services. Identify potential and realized barriers to care including transportation, childcare, and housing, then facilitate referrals to mitigate these obstacles. Develop holistic plans of care, expedite scheduling for appointments and diagnostic testing, and provide education to patients, families, and caregivers regarding the cancer healthcare system and multidisciplinary team roles. Participate in tracing metrics and patient outcomes to document and evaluate program effectiveness, assist in identifying candidates for molecular or genetic testing, and act as a liaison between patients and providers to optimize outcomes. Requires a BSN, three years of oncology experience, and Nurse Navigation Certification within two years of hire; 40-hour schedule with loan repayment up to $20,000 available.

Required Qualifications

  • Graduate from an accredited Nursing Program
  • Bachelor of Science Degree in Nursing (BSN)
  • Licensed Registered Nurse with New Hampshire eligibility
  • Nurse Navigation Certification
  • Basic Life Support (BLS)

Desired Qualifications

  • Master's degree
  • Three years of oncology experience in an acute or ambulatory care setting
