Job Summary
RN On-Call in-home care assignment in Detroit, MI area. The role involves after-hours skilled nursing care in patients’ homes, responding to post-acute needs from 4pm to 8am when on-call, and daytime hours 4pm–9pm. You will support a team of nurses and patients by delivering care at the top of your license, conducting OASIS assessments to tailor care plans, administering medications as prescribed, supervising nurse aides/LPNs, coordinating admissions/transfers/discharges, educating patients and families, ensuring accurate clinical documentation, and collaborating with the interdisciplinary team to achieve optimal outcomes. Mileage reimbursement and a competitive benefits package are offered, with additional bonuses such as a $15,000 sign-on bonus. The role requires a current RN license in the state of employment, and a minimum of one year of RN experience in an acute setting; home-care experience is preferred. A valid driver’s license and auto insurance are required, and a designated home workspace is expected for chart reviews when after-hours needs are minimal.
