Job Summary
RN On-Call provides skilled nursing care in patients' homes during after-hours, coordinating with an interdisciplinary team to develop individualized care plans, administer medications as prescribed, supervise aides/LPNs, and educate patients and families using teach-back methods. Responsibilities include OASIS assessments when indicated, chart reviews, and maintaining documentation in accordance with agency and Medicare guidelines, with a focus on reducing hospital readmissions and supporting patients in their preferred environment within the assigned territory.
Required Qualifications
- Registered Nurse with current license in the state of employment
- Minimum one-year experience as an RN in an acute care setting
- Home care experience preferred
- Valid driver's license and auto insurance in your name as a driver
- Designated workspace at home to assist with chart reviews if there are minimal to no after-hours needs
