Job Summary
Registered Nurse (RN) licensed in Idaho provides Home Health & Hospice care in Chubbuck with on-call duties (7 on 7 off). Conducts initial and ongoing patient assessments, develops and revises care plans, administers medications and treatments as prescribed, counsels patients and families, identifies discharge planning needs, coordinates care across disciplines, and acts as Case Manager when assigned. Maintains communication with primary physician, documents clinical notes, and collaborates with community health professionals to implement holistic nursing care.
Required Qualifications
- RN licensed in the State of Idaho
- Valid Driver's License
- Home Health and Hospice experience preferred
