Job Summary
Conduct remote nursing assessments and triage for new and ongoing members, including virtual care patients and in-person clinic visits. Support member check-out, scrubbing schedules for compliance, internal care team referrals, billing, and administrative tasks. Assist providers with virtual patient care by managing prior authorizations, scheduling labs, entering data into the EMR, and contacting patients. Process test results and provide timely updates via e-message, mail, or phone. Address social determinants of health by connecting members to resources for housing, food, transportation, and medication costs. Perform medical record requests and abstraction for virtual care patients while documenting interactions accurately. Work with members to identify barriers and remove them through community resource coordination.
Required Qualifications
- Completion of ASN or BSN with RN licensure
- Training as a Registered Nurse
- Experience with patient triage
- Ability to adequately use an EMR system
- Ability to navigate web-based platforms to generate reports, order labs, schedule appointments, etc.
Desired Qualifications
- Proficient in using Office 365 software, especially Outlook, Excel, and Word
- Ability to work independently and with a team in a virtual environment
- Ability to organize and multi-task
- Solid written and verbal communication skills
- Consideration of LPN with extensive experience
