Job Summary
As a Field Team Partner, conduct cognitive and functional assessments for older adults in their homes as part of CareScout's Field Team Network. You will decide, via a simple app, which assessments to take and how many, based on case opportunities and time/place considerations. Responsibilities include collecting information on medical history, documenting functional and cognitive levels, administering a short cognitive exam, guiding the insured through a brief demonstration activity, and completing assessments online and offline through the CareScout portal. The role requires travel within a 15-mile radius of your home, a valid RN license, and access to a laptop or tablet to perform assessments in real time. Training is completed online, and opportunities are accepted or declined via the portal. Work is contract-based, with fixed rates per assessment, and payments are tracked in the system.
Required Qualifications
- Current RN license in good standing
- Willingness to travel within 15 miles of home
- Access to a laptop or tablet to perform assessments in real time
- Ability to conduct cognitive and functional assessments in patients' homes
- Experience or capacity to document assessments online
- Contractor/PRN/Per diem work arrangement
