Registered Nurse (RN) - Nights

Provide comfort-focused in-home nursing care for patients with prognoses of six months or less, making 4-6 daily visits to private residences or assisted living facilities. Work on an interdisciplinary team of hospice clinicians, including LPNs, aides, chaplains, social workers, and physicians, while serving as an advocate for each patient and family to ensure goals of care are communicated. Deliver basic hospice education and training to patients, families, and caregivers. Once weekly, visit the local program office to collaborate with the interdisciplinary team on holistic support. Maintain a valid RN license and at least one year of hospital nursing experience.
