Registered Nurse RN Nights

Case manage patient needs in the field, performing ongoing assessments of physical, psychological, social, and spiritual requirements while initiating appropriate interventions. Provide and oversee supervision for LPNs, CNAs, and other assigned staff, taking independent action to identify problems and resolve potential issues proactively. Create and implement nursing care plans reviewed daily, collaborate with team physicians on patient status, and communicate regularly with management for risk management. Educate the community regarding hospice care philosophy through individual consultations and maintain positive working relationships with all disciplines. Complete required documentation prior to shift departure and actively participate in interdisciplinary groups to coordinate care.
