Job Summary
Provide hospice/home-health nursing care per physician orders; assess, monitor, and document patient symptoms; collaborate with patient, family, and interdisciplinary team to develop individualized plans of care; supervise aides/LPNs; participate in on-call rotating schedule; advocate for patient rights and education; maintain clinical documentation; ensure safety and quality of care in line with hospice/home health practices.
Required Qualifications
- Must be a Registered Nurse licensed in the state of operation.
- Nursing experience required, with experience in Hospice or Home Health preferred.
- Must possess a sound knowledge of nursing principles, procedures and elements of patient family teaching.
