Job Summary
Utilizes the nursing process to identify and achieve patient goals including assessment, planning, implementation, and evaluation. Assesses the physical, psychosocial, and environmental factors affecting patient health to develop comprehensive nursing care plans. Collaborates with the interdisciplinary team for optimal patient care and teaches patients/caregivers about healthcare and disease management needs.
Required Qualifications
- Unencumbered current Registered Nurse license in the state where the HomeCare agency is located
- Minimum one (1) year experience as a RN / professional nurse
