Formulate and evaluate individualized care plans in collaboration with healthcare professionals while coordinating resources to ensure readiness for patient discharge. Collect and document health data, identify variances in patient outcomes, and utilize read-back and escalation procedures appropriately. Support infection prevention and fall reduction through frequent rounding and evidence-based intervention. Work alongside experienced interdisciplinary teams across high-acuity patient populations to ensure optimal patient outcomes in a supportive and safe environment.
