Job Summary
Registered Nurse (RN) for Home Health & Hospice providing skilled nursing care in patients’ homes and hospice settings. Conduct comprehensive patient assessments; develop individualized care plans with physicians, patients, and families; administer medications, IV therapies, wound care, and other skilled nursing procedures; monitor and document patient progress; educate patients and families on disease management and home care; coordinate interdisciplinary care; ensure accurate EMR documentation; participate in on-call rotation; maintain HIPAA compliance; provide end-of-life care and emotional support to patients and families.
Required Qualifications
- Current and valid Registered Nurse (RN) license in the State of Texas (or compact state)
- Graduate of an accredited School of Nursing (BSN or ADN)
- Current CPR (BLS) certification
- Valid Driver’s License, auto insurance, and reliable transportation
