Job Summary
Registered Nurse (RN) needed for Home Health & Hospice to conduct comprehensive patient assessments, develop individualized care plans, administer medications and skilled nursing procedures, monitor progress, educate patients and families, coordinate with interdisciplinary teams, maintain EMR documentation, participate in on-call, and provide end-of-life care in home/hospice settings. Requires Texas RN license (or compact state), BSN or ADN, CPR/BLS, valid driver’s license, and reliable transportation. 1+ year clinical nursing experience preferred, with home health or hospice knowledge and EMR/OASIS familiarity highly valued. Must be able to travel to patient homes, work independently, and uphold HIPAA and regulatory standards while delivering compassionate, patient-centered care.
Required Qualifications
- Current and valid Registered Nurse (RN) license in the State of Texas (or compact state)
- Graduate of an accredited School of Nursing (BSN or ADN)
- Current CPR (BLS) certification
- Valid Driver’s License, auto insurance, and reliable transportation
