Job Summary
Registered Nurse (RN) for Home Health & Hospice providing in-home and hospice patient care. Responsibilities include comprehensive patient assessments, developing and implementing individualized care plans, administering medications and IV therapies, wound care, documenting in EMR, coordinating with interdisciplinary teams, on-call coverage, and delivering patient-family education with a compassionate, patient-centered approach. Requires Texas RN license (or compact state), BSN or ADN, CPR/BLS, valid driver’s license, and driving to patient homes; 1+ year clinical nursing experience preferred in home health or hospice; familiarity with OASIS and EMR charting preferred; travel to patient homes with flexible schedule including weekends and on-call duties.
Required Qualifications
- Current and valid Registered Nurse (RN) license in the State of Texas (or compact state)
- Graduate of an accredited School of Nursing (BSN or ADN)
- Current CPR (BLS) certification
- Valid Driver’s License, auto insurance, and reliable transportation
- Minimum one (1) year of clinical nursing experience preferred (home health or hospice experience strongly preferred)
- Knowledge of OASIS and EMR charting preferred for home health nurses
- Hospice RNs should have knowledge of symptom management, palliative care, and end-of-life protocols
- Strong clinical assessment and critical-thinking skills
- Excellent communication and interpersonal abilities
- Ability to work independently and manage time efficiently
- Proficient in computer systems and electronic documentation
- Strong organizational and problem-solving skills
