Job Summary
Registered Nurse (RN) – Home Health & Hospice role in Texas. Provides skilled nursing care to patients in homes or hospice settings. Conducts comprehensive assessments, develops individualized care plans with physicians and families, administers medications and IV therapies, wound care, and other procedures. Documents progress in EMR, participates in on-call rotation, and ensures HIPAA compliance. Educates patients and families on disease management and home care techniques; coordinates with interdisciplinary teams; supports end-of-life care and palliative care in hospice settings.
Required Qualifications
- Current and valid Registered Nurse (RN) license in the State of Texas (or compact state)
- Graduate of an accredited School of Nursing (BSN or ADN)
- Current CPR (BLS) certification required
- Valid Driver’s License, auto insurance, and reliable transportation required
- Minimum one (1) year of clinical nursing experience preferred (home health or hospice experience strongly preferred)
- Knowledge of OASIS and EMR charting preferred for home health nurses
- Hospice RNs should have knowledge of symptom management, palliative care, and end-of-life protocols
- Strong clinical assessment and critical-thinking skills
- Excellent communication and interpersonal abilities
- Compassionate, patient-focused, and empathetic approach to care
- Ability to work independently and manage time efficiently
- Proficient in computer systems and electronic documentation
- Strong organizational and problem-solving skills
- Working Conditions: Patient visits in home, assisted living, and hospice settings; Requires driving to patient homes and facilities; Flexible schedule including possible weekends and on-call duties
- Education & Licensure: Current RN license in TX; BSN or ADN; CPR/BLS; valid driver’s license
