Job Summary
Provide skilled nursing care to patients in their homes or hospice settings, conducting comprehensive assessments, developing individualized care plans with physicians and families, administering medications, IV therapies, and wound care, and monitoring patient progress. Educate patients and families on disease management and home care techniques; coordinate care with interdisciplinary teams; ensure accurate EMR documentation; participate in on-call rotations; maintain HIPAA compliance; deliver compassionate end-of-life care and emotional support in hospice settings.
Required Qualifications
- Current and valid RN license in Texas (or compact state)
- BSN or ADN from accredited School of Nursing
- Current CPR (BLS) certification
- Valid Driver’s License, auto insurance, and reliable transportation
- Minimum one year of clinical nursing experience preferred (home health or hospice strongly preferred)
- Knowledge of EMR charting (OASIS preferred for home health)
- Hospice RNs should have knowledge of symptom management and end-of-life protocols
- Strong clinical assessment and critical-thinking skills
- Excellent communication and interpersonal abilities
- Ability to work independently and manage time efficiently
- Proficient in electronic documentation
- Strong organizational and problem-solving skills
- Willingness to participate in on-call rotation
- HIPAA compliance
