Job Summary
The Registered Nurse (RN) will join the Home Health and Hospice team to deliver skilled nursing care in patients' homes and hospice settings. Key duties include conducting comprehensive patient assessments, developing individualized care plans with physicians and families, administering medications and IV therapies, wound care, and other skilled procedures, monitoring patient progress, providing patient/family education on disease management and home care, coordinating with interdisciplinary teams, ensuring precise EMR documentation, participating in on-call rotations, and upholding HIPAA-compliant practices. Hospice responsibilities include providing compassionate end-of-life care and supportive services. Requires TX RN license (or compact state), BSN or ADN, and CPR/BLS certification, with traveling to patient homes as part of home health visits.
Required Qualifications
- Current and valid Registered Nurse (RN) license in the State of Texas (or compact state)
- Graduate of an accredited School of Nursing (BSN or ADN)
- Current CPR (BLS) certification
- Valid Driver’s License, auto insurance, and reliable transportation
- Minimum one (1) year of clinical nursing experience preferred (home health or hospice experience strongly preferred)
- Knowledge of OASIS and EMR charting preferred for home health nurses
- Hospice RNs should have knowledge of symptom management, palliative care, and end-of-life protocols
- Strong clinical assessment and critical-thinking skills
- Excellent communication and interpersonal abilities
- Compassionate, patient-focused, and empathetic approach to care
- Ability to work independently and manage time efficiently
- Proficient in computer systems and electronic documentation
- Strong organizational and problem-solving skills
