Job Summary
Registered Nurse (RN) – Home Health & Hospice providing skilled nursing care in patients' homes and hospice settings. Responsibilities include conducting comprehensive initial and ongoing patient assessments, developing individualized care plans, administering medications and IV therapies, wound care, and other skilled nursing procedures; monitoring and documenting patient progress; educating patients and families on disease management and home care; coordinating care with interdisciplinary teams; ensuring accurate EMR documentation; participating in on-call rotations; maintaining HIPAA compliance; and providing compassionate end-of-life care and emotional support to patients and families.
Required Qualifications
- Current and valid Registered Nurse (RN) license in the State of Texas (or compact state)
- Graduate of an accredited School of Nursing (BSN or ADN)
- Current CPR (BLS) certification
- Valid Driver’s License, auto insurance, and reliable transportation
- Minimum one (1) year of clinical nursing experience preferred (home health or hospice experience strongly preferred)
- Knowledge of OASIS and EMR charting preferred for home health nurses
- Hospice RNs should have knowledge of symptom management, palliative care, and end-of-life protocols
