Job Summary
Provide skilled nursing care in home health and hospice settings, conducting initial and ongoing patient assessments, developing individualized care plans, administering medications including IV therapies, monitoring patient progress, educating patients and families, coordinating with interdisciplinary teams, ensuring HIPAA-compliant documentation in EMR, participating in on-call rotations, and delivering compassionate end-of-life care.
Required Qualifications
- Current and valid Registered Nurse (RN) license in the State of Texas (or compact state)
- Graduate of an accredited School of Nursing (BSN or ADN)
- Current CPR (BLS) certification required
- Valid Driver’s License, auto insurance, and reliable transportation required
- Minimum one (1) year of clinical nursing experience preferred (home health or hospice experience strongly preferred)
- Knowledge of OASIS and EMR charting preferred for home health nurses
- Hospice RNs should have knowledge of symptom management, palliative care, and end-of-life protocols
- Strong clinical assessment and critical-thinking skills
- Excellent communication and interpersonal abilities
- Compassionate, patient-focused, and empathetic approach to care
- Ability to work independently and manage time efficiently
- Proficient in computer systems and electronic documentation
- Strong organizational and problem-solving skills
