Registered Nurse (RN)

Provide skilled nursing care to patients in their homes or hospice settings, including comprehensive assessments, development of individualized care plans with physicians and families, administration of medications and IV therapies, wound care, and ongoing monitoring. Deliver patient and family education on disease management and home care techniques; coordinate patient care with interdisciplinary teams; maintain accurate EMR documentation; participate in on-call rotations; ensure compliance with HIPAA and agency policies; and deliver compassionate end-of-life care and emotional support to patients and families.
