Job Summary
Seeking a compassionate Registered Nurse (RN) for Home Health and Hospice teams. Responsibilities include conducting comprehensive patient assessments, developing individualized care plans, administering medications and skilled nursing procedures (including IV therapies and wound care), monitoring and documenting patient progress, educating patients and families, coordinating care with interdisciplinary teams, ensuring accurate EMR documentation, participating in on-call rotations, complying with HIPAA and agency policies, and providing compassionate end-of-life support in hospice settings. Requires Texas RN license (or compact state), BSN or ADN, CPR/BLS, valid driver’s license, and reliable transportation. 1 year of clinical nursing experience preferred; OASIS and EMR knowledge preferred for home health; hospice experience beneficial. Strong clinical assessment, communication, independence, organization, and problem-solving skills are essential.
Required Qualifications
- Current and valid Registered Nurse (RN) license in the State of Texas (or compact state)
- Graduate of an accredited School of Nursing (BSN or ADN)
- Current CPR (BLS) certification
- Valid Driver’s License, auto insurance, and reliable transportation
- Minimum one (1) year of clinical nursing experience preferred (home health or hospice experience strongly preferred)
- Knowledge of OASIS and EMR charting preferred for home health nurses
- Hospice RNs should have knowledge of symptom management, palliative care, and end-of-life protocols
