Registered Nurse (RN)

Conduct comprehensive initial and ongoing patient assessments, including physical, psychosocial, and environmental evaluations. Develop individualized care plans in collaboration with physicians, patients, and families. Administer medications, IV therapies, wound care, and other skilled nursing procedures as ordered. Monitor, record, and report patient progress and changes in condition to physicians and other care team members. Provide education to patients and families regarding disease management, medications, and home care techniques. Coordinate patient care with interdisciplinary teams, including therapists, social workers, and home health aides. Ensure accurate and timely documentation in the Electronic Medical Record (EMR). Participate in on-call rotation as assigned. Maintain compliance with all state, federal, and agency policies, procedures, and regulations, including HIPAA. Provide compassionate end-of-life care and emotional support to patients and families in hospice settings. Requires a valid Texas RN license, BSN or ADN, and current CPR certification. Flexible schedule including possible weekends and on-call duties with competitive pay and mileage reimbursement.
