Job Summary
Assess patient needs, develop care plans, and administer treatments for home health and hospice patients under physician direction. Monitor patient progress, perform skilled procedures including wound and IV care, and educate families on disease management and comfort strategies. Coordinate with interdisciplinary teams, ensure regulatory compliance, and provide compassionate end-of-life support and emotional assistance. Complete accurate documentation and respond to emergencies as required. Full-time, part-time, or PRN schedules with on-call rotation available.
Required Qualifications
- Graduate of an accredited school of nursing
- Current and active Registered Nurse (RN) license in the State of Texas
- Minimum 1 year nursing experience
- Home Health and/or Hospice experience
- Experience with wound care, IV therapy, and patient assessment
- Strong clinical assessment skills
- Excellent communication and interpersonal skills
- Ability to work independently
- Knowledge of Medicare and home health regulations
- Compassionate and patient-centered care
- Basic computer and EMR documentation skills
- Time management and organizational skills
- Current CPR/BLS certification
- Valid Texas Driver License and reliable transportation
- Auto insurance
- Ability to stand, walk, bend, lift, and assist patients
- Ability to travel to patient homes
- Ability to lift up to 50 pounds occasionally
- Full-time / Part-time / PRN availability
- On-call rotation availability
Desired Qualifications
- Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN)
- OASIS documentation experience
- Kinnser/WellSky EMR experience
- Bilingual (English/Spanish or other languages)
