Job Summary
Registered Nurse (RN) providing direct and indirect nursing care to participants following care plans, with ~20% home visits. Responsibilities include collecting and analyzing patient data, developing and evaluating plans of care with interdisciplinary teams, providing education and discharge instructions to participants and families, responding to emergencies, documenting care in the EMR, and participating in Quality Improvement initiatives. Requires travel to participant homes, strong communication, ability to work independently within a fast-paced healthcare setting, and CPR/First Aid certification (or willingness to obtain after hire).
Required Qualifications
- Graduate of an accredited nursing school
- Current RN license in practicing state
- CPR and First Aid Certification (or willingness to obtain after hire)
- Valid State driver’s license with clean driving record
- Successful completion of background check, physical, drug screening, TB screening
