Job Summary
Provide skilled nursing visits in accordance with physician orders, completing start of care, evaluation, follow-up, and routine visits. Monitor patient conditions, report changes to the clinical team, and assist with medication management, wound care, and patient education. Complete accurate clinical documentation and coordinate care to ensure continuity. Maintain compliance with home health standards and regulatory requirements while working independently in the Davenport/Polk County area. Per diem opportunities available with flexible scheduling based on patient needs.
Required Qualifications
- Active Florida RN or LPN license in good standing
- Valid driver's license
- Reliable transportation
- Ability to work independently in the field
- Strong clinical assessment and documentation skills
- Professional, dependable, and patient-focused
Desired Qualifications
- Home health experience
- Experience with skilled nursing visits
- OASIS / Start of Care experience for RN candidates
