Registered Nurse (RN) - Lake Charles - PRN

Home Health RN lead role conducting in-home assessments, developing individualized care plans, delivering skilled nursing care, coordinating across disciplines, and educating patients and families to support disease management and independence. Responsibilities include case management across therapists and physicians, care transitions, accurate EMR documentation, supervision of Home Health Aides, and participation in on-call rotations within assigned Basile, LA territory. Requires strong clinical judgment, independent decision-making, effective communication, and ability to manage a caseload while delivering high-quality patient-centered care.
