Registered Nurse (RN)

Hospice RN Case Manager responsible for initial patient/family assessment to determine hospice needs and develop an individualized plan of care focused on comfort and quality of life. Coordinates with the interdisciplinary team, supervises LPNs and hospice aides, provides hands-on support for pain and symptom management, and spends meaningful time with patients and families. Monitors changes in condition, carries out physician orders, adjusts the plan of care as needed, participates in on-call rotation, and performs other duties as assigned.
