Monitor, examine, and evaluate patient conditions while assessing, planning, implementing, and evaluating care to promote short- and long-term outcomes. Collaborate with the care team to coordinate daily patient flow, deliver safe care according to policies, and maintain accurate documentation of assessments and incidents. Listen to patient concerns, serve as an educational resource on health issues, and communicate pertinent information to families and providers to maintain satisfaction. Participate in performance improvement initiatives, seek opportunities to improve processes, and contribute ideas to support change implementation.
