Job Summary
Conduct telephone triage, anticoagulation management, and nurse-only visits for adult patients, including patient teaching, wound care, and medication agreements. Manage acute and chronic conditions within a collaborative team focused on improving day-to-day health and wellness. Work 8-hour shifts from 7:30am to 5pm Monday through Friday as a part-time 0.8 FTE role. Requires RN licensure, BLS certification, and prior nursing experience.
Required Qualifications
- Associate Degree in Nursing
- RN licensure to practice in the state of Wisconsin
- RN licensure for Iowa and Minnesota within 3 months of hire
- BLS for Healthcare Providers
- RN experience
