Job Summary
Provide home-based care to patients with illnesses, injuries, and chronic diseases, often following hospital or rehab discharge. Examine patients, conduct assessments, document progress, and report changes to physicians. Educate patients on care plans, medications, therapy, diet, and exercise. Coach family members on patient plans, medication management, and home safety. Coordinate discharge goals and work with a team of physicians to oversee LPNs, CNAs, and aides. This role requires an active Colorado RN license, 1,200 hours of RN experience, and BLS certification. Minimum expectation is five visits or ten hours weekly, with potential to build to full-time. Required Qualifications
- Active Registered Nurse (RN) License in Colorado
- Graduate of an accredited nursing school or program
- At least 1200 hours of experience as an RN
- Health care CPR/BLS Certified (demonstration course required)
- Pass a background check and drug test (we are required to test for THC)

- Home health care experience preferred
