Job Summary
Provide home-based care to patients with illnesses, injuries, and chronic diseases, often following hospital or rehab discharge. Examine patients, conduct assessments, document progress, and report changes to physicians. Educate patients on care plans, medications, therapy, diet, and exercise. Coach family members on home safety and care protocols. Coordinate discharge planning and ensure goals are met. Work with a team of physicians and supervise LPNs, CNAs, and aides. Maintain CPR certification and knowledge of state and federal home health regulations.
Required Qualifications
- Graduate of an accredited nursing program
- Active RN license in Kansas
- CPR certification
- Knowledge of state and federal home health regulations
- Clinical proficiency in nursing specialty
- Strong communication skills
- Compassion
