Registered Nurse (RN)

Perform comprehensive nursing assessments and skilled visits, including medication management, wound care, and disease management. Develop individualized plans of care and complete Start of Care, Resumption of Care, Recertification, and Discharge assessments. Monitor patient progress, communicate changes to physicians, and coordinate care with therapists and home health aides. Educate patients and caregivers to promote safety and independence while documenting all activities in WellSky.
