Registered Nurse (RN)

Perform comprehensive patient assessments, develop individualized care plans, and provide skilled nursing services including medication management, wound care, and patient education in home settings. Monitor patient progress, communicate updates to physicians, and coordinate care with therapists, social workers, and home health aides. Complete timely clinical documentation and educate patients and caregivers to promote health, safety, and independence.
