Registered Nurse (RN) Hospital Liaison

Coordinate patient healthcare services and determine home care eligibility by reviewing medical documentation and insurance. Attend discharge and multidisciplinary rounds to assist with transitions from acute care to home settings, ensuring all ancillary services are coordinated per patient needs. Build positive relationships with physicians, facility personnel, and patients while serving as a resource for care conferences and case management. Review clinical documentation against Medicare and Federal guidelines to maintain compliance.
