Registered Nurse (RN) -Hospital Case Manager, PRN, Days

Conduct discharge planning for patients with complex medical issues to ensure timely transitions to appropriate post-acute care providers. Collaborate with physicians, interdisciplinary teams, and payors to resolve care progression barriers and address system impediments to diagnostic or treatment progress. Assess psychosocial risk factors and advocate for patient empowerment while maintaining expert knowledge of body systems and evolving state and federal regulatory requirements. Provide clinical reviews to third-party payors, negotiate healthcare resources, and educate patients on care options throughout the continuum.
