Identify social determinants of health risks, coordinate post-acute plans with ambulatory teams, and resolve system obstacles impeding diagnostic or treatment progress. Navigate value-based care requirements, third-party payors, and complex regulatory compliance while advocating for patient autonomy and access to necessary services. Assess psychosocial risk factors through evaluation of support systems and ability to cope, ensuring clear documentation for the interdisciplinary care team. Maintain expert knowledge of body systems, clinical outcomes, and state/federal regulatory requirements to drive performance improvements and align longitudinal care plans with patient goals.
